About the Role
This is a new role for Focused on Care and will be the primary contact for their portfolio of Home and Community clients and the support workers that work with those clients. You will be supported directly by the Service Delivery Coordinator (Home and Community Supports) to ensure our clients are receiving the highest quality care and to ensure out Support Workers are supported effectively to deliver this care.
