Qualifications, Skills and Knowledge
The successful candidate will have tertiary qualifications in Community Services, Social Work, Psychology, Occupational Therapy or equivalent. They will also be required to have completed the NDIS Worker Orientation Module “Quality, Safety & You” and any other mandatory training as advised by BCOGNITIVE.
The successful candidate will have a strong knowledge of the NDIS and support coordination and a knowledge of relevant service providers across mainstream and other community service sectors, including mental health and disability providers.
The successful candidate should be proficient in Microsoft Office, particularly MS Outlook, Word and Excel.
Responsibilities
The successful candidate's duties may vary depending on the specific needs of the participant and the goals outlined in their NDIS plan. Typical day-to-day responsibilities of the position may include but are not limited to:
- Being responsible for providing comprehensive support coordination to participants with complex needs, behaviours of concern, medical needs, or other high-risk circumstances or where the National Disability Insurance Agency has deemed it reasonable and necessary to fund the Specialist Support Coordination funded support in their plan.
- The Coordinator works with these participants to resolve points of crisis, assists in risk management procedures, provides informed choice and decision-making to the participants about a wide range of services and supports, interfaces with mainstream and community services and ensures a high quality of integration of service delivery for these participants.
- Support NDIS participants in implementing their NDIS plan to maximise the value of the funding they receive from their support.
- The Coordinator may provide a referral-specific service as requested by the participant’s Planner and a case-specific specialist support coordination report that offers a comprehensive report for the NDIS planner.
- At all times, exercise the client’s choice and assist and coach participants using the NDIS participant portal.
- Develop trusting, positive, professional relationships with Participants and service providers. Work with Participants concerning each individual’s right to dignity, privacy, independence, and respect.
- Using a coaching approach to assist customers in capacity building.
- Support participants in facilitating their independence by enabling empowerment and choice and the promotion of personal resilience and social inclusion.
- Create and maintain accurate and up-to-date case notes.
- Undertake NDIS reassessment in line with Client NDIS plans.
- Implementing a continuous improvement process in all operations in line with the organisation’s directives.
- Ongoing attraction of new participants ensuring ongoing growth of client caseload Ensure that you are proactively contributing to the ongoing development of your individual skill set and capability and contribute to the capability and development of other team members as appropriate.
- Completion of annual performance and development plan.
- Knowledge sharing and coaching of others as requested.
Qualification & Experience
- Bachelor in Community Services or Social Work (or other allied health-related fields)
- Extended knowledge about NDIS and how to implement NDIS in a Mental Health environment.
- Ability to provide support, facilitating the work of others (not managing them) that is tailored to the need’s teams and individuals.
- Willing and able to exercise personal responsibility.
- Demonstrated computer literacy to use Microsoft programs and the capability to learn other applications.
- Understand the needs of hard-to-reach communities, CALD, Aboriginal communities and the LGTBQIA community.
- Work within ODMH policies and protocols.
