Services provided by the Newcomb Community Health Centre include podiatry, physiotherapy, psychology, social work, nutrition, speech pathology, community health nursing and occupational therapy. Barwon Health Community Nursing is a HACC funded service and also provides care for clients requiring Hospital in the Home, Post-Acute and Palliative care.
About the roleWe are seeking an experienced and motivated Clinician with a strong
Primary Care background to fill a full time permanent role.This role will lead the
Primary Care Health Team, based in the Eastern Suburbs of Geelong. This role is part of the broader East Area Health Team, focusing on delivering high quality care, linking into a range of services delivered from the Newcomb Community Health Centre and locally in our community.This role will coordinate daily operations of the team and assist in planning, implementation and organisation of care aligned with the model. It will ensure that the team strives to achieve excellent health outcomes for the community, by providing person centred and integrated service that enables efficient and effective care delivery.Working in partnership with services at their site, will see the Coordinator be proactive in identifying opportunities, which support partnership approaches to health delivery.About the cultureThere is a strong and supportive team culture in our services.
