At Advanced Support Services, we are dedicated to improving the lives of individuals with disabilities, and we are currently seeking a passionate and motivated individual to join our team as a Service Coordinator.
Roles and Responsibilities:
- Legislative framework Application: Apply an understanding of the relevant legislative frameworks to day-to-day work practices in supporting the Company as a whole.
- Services Supervision: Ensure services are provided in ways that reflect the underpinning philosophies and principles for supporting people with a disability.
- Document Management: Ensure all necessary documents are visible, completed, and filed.
- Clients Visitation: Visiting clients on a regular basis.
- Client Onboarding and Exiting: Meeting with new clients, and managing the onboarding and exit process of other clients.
- Completing Necessary Assessments: Conducting risk assessments, and completing needs assessments for new clients.
- Recruitment: Actively participating in the recruitment and onboarding process of new employees.
- Team Support: Support the Disability Service team members, including developing and implementing care plans and projects and enhancing team cohesion.
- On-call: To be available for on-call duties as directed by the organization. To always remain available and contactable by phone while on call.
- Flexibility: To be flexible to perform other related tasks as the organisation requests when the person in charge is unavailable.
