- Minimum 6 months experience as a Service Coordinator or similar role within similar Industry
- Hold a Certificate IV in Community Services or similar Qualification
- Experience with complex case management, Supported Independent Living, and interagency liaison
- Ideally lives in the Southern part of the River
- Demonstrated knowledge and experience in disability program management (assessment, coordination, community linking and budgets) and understanding the care needs of people within the disability sector;
- Commitment to person centered care and quality of care;
- Demonstrated ability to supervise and lead staff;
- Demonstrated ability to plan, co-ordinate and organize own work and work of team members;
- Demonstrated ability to develop a customer service culture amongst staff;
- Ability to maintain professionalism, integrity and confidentiality;
- Proven ability to plan, prioritize, meet deadlines and juggle competing demands, working in a fast paced and flexible environment;
- Proven ability to document progress, and address barriers when delivering services;
- Proven ability to respond to crises and incidents as required;
- Strong knowledge of the Disability Services and NDIS Act requirements;
- Hold a current Driver's License
- A road worthy vehicle with comprehensive insurance
- Hold a current Working with Children Check for paid employment
- A Certificate of Completion of Worker Orientation Module
- NDIS Worker Screening Module
- Hold a Certificate IV in Community Services or similar qualification
- Australian or New Zealand citizenship or a relevant working visa
- Will have to report to the D360 Supports Main Office
