The Opportunity:
Due to a period of exciting growth, My Choice Care is looking for a Team Leader (Home Care) to support the In-Home Care Manager in achieving person-centered care practices for clients.
Working from our Fullarton office, the Team Leader (Home Care) will work collaboratively and lead a team of Service Consultants to support elderly community members living at home to reach their goals and maintain their independence.
Key Duties:
- Provide appropriate resources, leadership, direction, support, and coaching to the team of Service Consultants.
- Maintain management systems that ensure standards of quality care.
- Oversee the schedule of client assessments and re-assessments.
- Monitoring the quality of comprehensive contemporary assessment and care planning practices.
- Deal constructively with conflict and mediate and negotiate acceptable outcomes.
Essential Criteria:
- Current Driver’s License.
- Current Police Clearance.
- Sound understanding of the Home Care Package Program.
- Experience managing and leading a team in a Home Care setting, particularly through periods of change in practice, culture, services, and approaches to Home Care.
- Medium to high level of computer literacy and ability to adapt to client and quality management systems.
- Well-developed time management, problem-solving and decision-making skills.
- Proven ability to think and act strategically, setting realistic goals and plans that improve services.
Desirable Criteria (non-essential):
- Previous experience managing a case load in the Health Care Sector (Aged Care, Nursing, Disability Care).
- Experience working with CALD communities.
- Tertiary qualification in Business, Management, or Aged Care (Individual Support).
