The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
IUIH Goodna’s aim is to improve access to the most appropriate health and social services for Aboriginal and Torres Strait Islander people at the right time and in the right place with a view to reducing avoidable acute hospital admissions or re-admissions and emergency department presentations; improved use of the healthcare system and to support people to take part in a healthy lifestyle and support clients to access social supports including safe accommodation.
About the Role?
This position will be an exciting opportunity for the right person to work directly and effectively with young people between the ages of 12 - 24years to develop positive pathways that will create a supportive network for social & primary health care and will work with families, carers, friends and their community.
The position will also work collaboratively with a range of organisations and primary health care providers to coordinate, improve, change and integrate services to meet the needs of young people.
Your day-to-day duties may include:
- Work alongside young people and families to develop effective pathways and networks to support young people and their families / carers.
- Work with clients to Identify goals for their health and well-being
- Participate in, and / or coordinate, multidisciplinary care conferencing to enhance support coordination for our clients.
- Support clients to access primary health care services and to establish and / or maintain continuity of care through a regular ACCHS or mainstream GP service in the community.
- Coordinate and support access to required specialist, allied health, mental health, and social health services, including child protection and related services and supports, as indicated in the client and family’s plan
About You?
- Current Criminal History Check or willingness to obtain
- Certificate IV or higher in Health or Social Services related field
- Current C Class Drivers Licence (Qld)
Desirable, but not mandatory:
- Demonstrated experience working with Aboriginal and Torres Strait Islander people and /or communities
