Our teams are forever growing, and we are so thrilled to be on the hunt for our next experienced and passionate Aged CareWellbeing Manager, to join our ever-growing Darling Downs, QLD Team. Covering Toowoomba and Southern Downs area- Toowoomba, Clifton, Allora and Warwick.
Your Role:
You will be out and about in the Community, Ensure a high level of care is delivered in accordance with the client's individual care needs, organisational policies and evidence based best guidelines. You will provide a range of services to our valued clients, including:
- Help your clients to remain at the centre of their journey
- Develop HCP care plans and coordinate a multi-disciplinary team (MDT) for the delivery of in-home therapy, services, and equipment
- Manage & Coordinate the day to day delivery of care for HCP clients including managing their individualized budgets.
- Be the advocate for the client and participate in case conferencing and scheduled appointments as required.
Salary and benefits:
- Salary Banding: $78,507 - $98,869 (Depending on the level of your experience) + 11% plus super.
- Salary packaging, up to $15,899 tax-free annually.
- Meal and Entertainment Card, up to $2,650 of your pre-tax income per annum.
- Daily Travel allowance $25 p/day or Company Car options.

About You:
- Tertiary qualifications in Case Management (would be an advantage)
- Previous Case Management Experience (Community and Aged Care experience highly desired)
- Experience working with clients conducting HCP assessments and applying goal-oriented, outcome focused and holistic approaches.
- Demonstrated ability to develop and maintain strong networks with internal and external stakeholders.
- Current drivers licence and be comfortable traveling throughout the region each day
