Are you passionate about making a difference?

We are looking for an experienced Case Worker to join our Thrive team to be responsible for the assessment and case management of families and individuals (where there are complex needs including mental health, disability, drug and alcohol and child protection issues) to prevent homelessness, increase self-reliance and build capacity to live independently.

Our ideal applicant will have:

  • Degree in community services, community development, early childhood education or relevant experience
  • Demonstrated experience working with families where substance abuse, domestic/family violence, child protection and/or mental health issues are present
  • Demonstrated experience of working with Aboriginal families and families of diverse backgrounds
  • Demonstrated ability to work within a case management model to achieve positive client outcomes
  • Ability to assess and analyse difficult situations and recommend strategies for solutions
  • Sound understanding of Workplace Health and Safety
  • Demonstrated ability to build relationships, liaise and advocate effectively with a range of community stakeholders in the delivery of a service
  • Capacity for resilient, robust and autonomous work with a challenging client group
  • Demonstrated high level interpersonal skills and good written communication skills
  • Current Working with Children Check
  • Current satisfactory National Police Clearance
  • Current WA Driver’s licence

The Role

The key responsibilities of this role include:

  • Provide professional assertive assessment and case management for individuals and families in public housing tenancies to prevent homelessness, increase self-reliance and build capacity to live independently
  • Work with the family to build a client action plan (CAP)
  • Provide supportive counselling and education on tenancy rights and responsibilities, and the practical functioning of tenancy legislation
  • Provide a range of support interventions within a developmental framework that enables clients to address their needs, maintain their tenancy and enhance their social functioning
  • To ensure the use of the Thrive Program Activity Tracker to monitor work outputs and performance using best practice approaches
  • Advocate on behalf of individuals and liaise regularly with service providers
  • Practice self-care strategies and apply safety procedures in all work practices
