Home Caring is private home care company that provides aged and disability care across NSW, Victoria and Queensland through the NDIS and Home Care Packages.

We are committed to delivering sensitive and respectful person-centred care, offering a wide range of services such as personal care, meal preparations and domestic assistance in the comfort and privacy of the clients' homes. We also provide Day Programs, Capacity Building, Community Access and transport. 
 

About the Role

Home Caring Smithfield are seeking proactive and highly organised Support Coordinators, who enjoy engaging with people and ensuring high outcomes for participants and their funding. 

Our Support Coordinators:

• Support participants to learn about how the NDIS works and how to utilise their funding
• Are highly engaged and have in-depth knowledge of services available for referral and connection
• Support the participant with freedom of choice through their service provision
• Assist with executing Service Agreements
• Ensure budgets are adhered to, and advocate for the participant where funds are not sufficient for their support needs
• Find opportunities for participants to continue to engage in their communities
• Help participants through times of crisis (hospitalisation/ housing support etc)
• Have strong report writing/computer skills to ensure that participants have the best opportunities for plan reviews
• Coordinate services to provide reports for plan reviews.
• Have an excellent understanding of the NDIS Framework, including the Quality & Safeguards Commission

You must have:

• WWCC
• NDIS worker screening check or have applied and pending 
• Ability to travel and have own car
• Disability or Community Sector qualification
