In this role you will support participants to understand all aspects of their NDIS plan, as well as assist participants to navigate the NDIS provider network, sourcing & implementing the required services and supports to facilitate progression towards achieving their goals. You will also strive to effectively communicate & network with other government, community & mainstream services to refer your participants, promote Wellways NDIS services, increase new participant referrals in your region.Your working day will involve the interchanging provision of supports to participants, from establishing and connecting supports & services, monitoring plan budgets & support effectiveness, designing support approaches and encouraging skills development, promoting and supporting capacity building…
