Practice Nurse – En/Rn

Position Summary:

Hall Road Medical Centre is seeking a dedicated and experienced Practice Nurse – EN/RN to join our dynamic healthcare team. As a Practice Nurse, you will play a crucial role in providing high-quality nursing care and support to our diverse patient population. This position offers a rewarding opportunity to contribute to the delivery of comprehensive primary healthcare services. We offer a friendly supportive environment in which Practice Nurses work closely with GP's to deliver high quality patient care.

Key Responsibilities:

1. Conducting patient assessments, including vital signs, health history, and medication review.

2. Assisting with chronic disease management and coordinating care plans for patients.

3. Administering vaccinations and immunizations as per the guidelines.

4. Performing wound care, dressings, and other nursing procedures under the direction of medical practitioners.

5. Collaborating with other healthcare professionals to ensure seamless patient care.

6. Conducting health promotion and education sessions for patients.

7. Managing and maintaining accurate medical records and documentation.

8. Assisting with infection control procedures and maintaining a safe clinical environment.

9. Participating in team meetings and contributing to the continuous improvement of clinical processes.

10. Keeping abreast of relevant clinical guidelines and participating in professional development activities.

Qualifications and Requirements:

1. Registered Nurse qualification with current registration with the Australian Health Practitioner Regulation Agency (AHPRA).

2. Experience in a general practice or primary care setting is preferred.

3. Competence in conducting health assessments and delivering evidence-based nursing care.

4. Strong interpersonal and communication skills.

5. Ability to work effectively in a collaborative team environment.

6. Familiarity with relevant accreditation standards and commitment to quality improvement.

7. Current CPR and First Aid certification.

8. Knowledge of Medicare and government health initiatives.

9. Good working knowledge of Best Practice software.

This is a part-time position; however full-time role is considered in the near future. Hours of work 9.30 AM to 4.30 PM Mon-Frid.
